[IPAC-List] Critical incidents

I don't know about other places, but we allow respondents to provide
"comments" at the end of our government-wide survey on staffing
practices.  Although we do not expressly solicit such incidents, when
they do happen just TRY and stop people from telling you all about
them!
 
So, another potential source of critical incidents might be things like
employee survey comments.  Or for that matter, customer survey comments,
or the "complaints" department.
