Hi Eileen,

Building on John's idea, you might get some ideas for situations by
contacting people working in the Employee Assistance Program or by
talking with counsellors or ombudsmen/women. It would, of course, be
necessary to be careful about info that could possibly identify
individuals.

I've seen help desk records be quite useful for some jobs. Both as a

source of problems and solutions and as materials for subject-matter
experts
to review to produce critical incidents.
